Plan to arrive 10-15 minutes before your scheduled time; bring insurance card(s), State ID, a list of medications, and any referral or prescription if ordered by doctor.

Yes. We treat patients with work-related injuries and can assist with contacting your workers compensation carrier to ensure claim is open and billable.
