What conditions do you treat?
Sports related injuries, neck and back injuries, pre/post-surgical rehabilitation, overuse/repetitive stress injuries, hip/knee/foot/ankle injuries, shoulder/elbow/wrist injuries.
Do I need to bring forms to my first appointment?
If you are new, you can download the patient information forms and complete them prior to arrival, or you may arrive 15-20 minutes early to complete paperwork in the clinic.
What should I bring to my first visit?
Insurance card, photo identification, physician referral if necessary, applicable co-pay or deductible, wear loose clothing.
What insurance plans are accepted?
We are contracted with most insurance companies. A list includes Regence, Premera, Aetna, First Choice, Cigna, Labor and Industry, Worker’s Compensation, and various others; some plans like United HealthCare or Medicare may not be accepted.
Do you accept workers' compensation or motor vehicle accident claims?
Yes, we can handle Worker’s Compensation/L&I and motor vehicle accident (PIP) scenarios.
