- Fax HMD’s mobile order form (provided to the facility) to 573-221-8463 or log onto the physician portal (once you have registered and been given access) and place the order. - Call 573-221-8463 to ensure the fax/order was received. - Our Radiology Technologist will come to your facility to perform the services. - Once the radiologist has read the exam, a report will be faxed to your facility and to the ordering physician.
