Are you accepting new patients?

Yes, our physicians welcome new patients. However, those needing hemodialysis access or a vascular surgery consultation require a referral from their doctor. This process ensures appropriate care, insurance authorization and seamless appointment scheduling. Your primary care provider or physician can assist in coordinating the referral for you.

How can I access my medical records?

Call us at(423) 654-7670 to request a medical release form. A signed, completed release form is required, and a fee may apply.

What insurances do you accept?

We accept most major insurance plans. Check your policy to confirm coverage so we can submit the claim correctly. All insurance copays, coinsurance and deductibles are due at the time of service. Some insurance plans cover annual physicals.
