What should I bring to my first visit?

Please bring your license and insurance card to every patient visit. Also bring every medication you are taking (prescriptions, supplements, creams, and over-the-counter drugs) to ensure your medication list is accurate.

How early should I arrive for my appointment?

Please show up no later than 15 minutes prior to your scheduled visit time.

How do I access the Patient Portal?

Accessing the Patient Portal is easy and can be done in two ways: sign up for the Patient Portal through our website (link at the top of the home page) or use the computer in the office waiting room if you do not have internet access; a paper copy of all patient forms can also be requested.

What information should I submit before my appointment?

Complete all portal medical history, demographics, and necessary forms, or submit paper copies at least two days prior to your appointment.

EMR enables you to have control and accessibility to various systems to manage your health care and connect with your primary care provider online.

Can I have someone assist with data entry or forms?

Yes, staff can assist with Portal access; if needed, you may visit the office or use a local library for internet access.
