Do you require a referral for new patient visits?

Yes, a referral is required for all new patient visits.

An appointment is required for all visits.

If you need to cancel your appointment, we ask that you give us at least 24 hours’ notice. Not showing up for an appointment may result in a no-show fee or dismissal from the practice.

What should I bring to my diabetes visit?

- If you have diabetes, bring your glucometer and/or home blood sugar glucose log - Outside test results related to why we are seeing you - Current medication bottles or a detailed list, including doses and timing - Insurance card and photo ID

Can refills be authorized after hours?

We cannot authorize refills if you have not been seen for a planned office visit. Prescription requests will not be accepted after hours except for medical emergencies. Please allow 72 hours for your provider to approve refills outside office visits.

How can I access my medical information online?

Access your medical information online through MyChart, our secure online patient portal. Request an activation code online or during your next appointment.

What should diabetes patients expect in after-visit summaries?

For our diabetes patients, we will give you written instructions regarding what to do for high and low blood sugars in your after-visit summary, which can also be viewed on Summit Health MyChart.
