Initial appointments how scheduled?

Initial appointments may be scheduled by telephone with clarification of address, phone number, billing information, PCP, and referral requirements.

Insurance referrals

If your insurance requires a referral, obtain prior to appointment; without it we cannot provide services at the time of appointment.

We will need to clarify information such as address, phone number, billing information, your primary care physician and whether a referral is required.

If your insurance requires a referral, we ask that you obtain this prior to your scheduled appointment.

Without this referral we will be unable to provide services to you at the time of your appointment.

We will try to schedule a follow-up appointment at the end of every visit with us.

If you call in for an appointment we may need to refer you to the nurse to determine the type of visit required.
